About this Opportunity

As a member of the Resurrection Re-entry team, provide coordination of care for clients, engages community resources, providers, and neighborhood organizations to become involved in reentry programs; educates and assesses needs of clients to identify the appropriate community, governmental or private programs and services; responds to requests for services and recommends programs and interventions; assists individuals in obtaining appropriate services; develops procedures for new services to be utilized; oversees the coordination of new services for the client and acts as a liaison with program authorities regarding issues and coordination of services. * Conducts and facilitates case review meetings and Integrated Case Management Team meetings; makes logistical arrangements and provides documentation. Consistently collects and inputs client data into the database.

* Recruits, develops training and compliance check protocols, and coordinates volunteers and mentors; works with clients as they transition into a pro-social lifestyle. Assists with providing community outreach, and developing, coordinating, and implementing comprehensive reentry programs and objectives, and educational and health programs for clients; * Establishes and maintains positive working relationships with clients, CIRV and Reentry teams, volunteers, and mentors, as well as area hospital officials to engage victims of violence in reentry programming; works with, and maintains, confidential data and information. * Demonstrates regular and predictable attendance. * Reviews and assesses information, statistics, data and narratives regarding type, nature and frequency of services delivered. * Monitors timeliness, quality of service delivery, and sponsor activities. * Performs other duties as assigned

Requirements (Education, Experience, Licensure, Certification)

* Bachelor's degree in Social Work, Human Services, or related field and three (3) years experience in a social services capacity with direct emphasis on working with disadvantaged population in corrections or treatment environments using governmental, community, or private support service agencies employing a collaborative manner; or * equivalent combination of education and experience which evidences an advanced knowledge of accessing social services and governmental, community, or private human support service agencies for the disadvantaged population in corrections or treatment environments

This is a remote/virtual position however, the preferred candidate will live/work in the United States.
