About this Opportunity

Description:

Continuum Care Hospice is looking for the right person with excellent communication skills to make "check in" calls to our patients' families on a weekly basis. You will track supplies and medications that need ordering as well as to solicit feedback on patient and family satisfaction with overall hospice service delivery. A caring, sensitive demeanor is essential for this role. You will be trained on all elements to successfully carry out these responsibilities. You will be a part of a small, friendly and supportive environment, and your work here will be appreciated by the entire team as well as the hospice patients and their families. It is a great way to give back to your community.

The schedule can be flexible--days and times. This position can be performed virtually or take place in our office in Petaluma. 

Qualifications:

Understanding and supportive of hospice philosophy.

Compassionate person who is accepting and respectful of people's differences.

The ability to communicate well, both verbally and in writing.
