Role Overview
As a CHW Coordinator, you will help advance HHP’s mission to improve health literacy and empower underserved communities by leading a team of Community Healthcare Workers (CHWs) and supporting patient advocacy programs. This role is perfect for a motivated CHW looking to transition into program management and administrative oversight.
This is an entirely volunteer, remote-only position with flexible hours, giving preference to U.S.-based candidates.
Role Balance: This role is focused on Coordination/Program Management (Approx. 70%) with a direct service/patient advocacy component (Approx. 30%).
Duties (Reporting to the Program Director/Patient Advocate Lead)
- Team Coordination: Coordinate and track multiple CHWs and patients within an electronic referral system.
- Program Consulting: Provide expert consultation on initial CHW program creation and non-profit collaborations.
- Direct Service: Create connections between vulnerable populations and healthcare systems, including providing culturally appropriate health education.
- Reporting: Continuous evaluation of the CHW program and progress, with monthly data report creation (using Clockify, Trello, etc.).
Qualifications
Required:
- CHW Certification or commitment to complete training within 6 months of onboarding.
- Worked experience in community health or healthcare.
- Strong communications skills.
Preferred:
- Prior management or team lead experience.
- Ability to communicate effectively in other languages (Spanish, Mandarin, etc.).
- Experience with electronic patient referral systems.
Skills
- Ability to coordinate volunteers and assist with their training.
- Willingness to learn how to use and train new volunteers in health data and referral systems (Pear Suite, Unite Us).
- Knowledge of community health concepts, advocacy, care coordination, and health literacy.
- Ability to translate complex medical information for laypersons.
Conditions of Volunteer Acceptance
- Commitment: We ask for a commitment of ~7 hours per week for at least one year to ensure continuity of care and program impact. Flexibility is available to accommodate personal needs.
- Must track hours/tasks using HHP's required tools (Clockify/Trello).
